Emergency phone dialer board?

Does anyone know of a chip or board that implements an emergency dialer? The idea is that if I generate a logic High, I want a board to wake up, detect dial tone, call a pre-specified number, wait for pickup, play a short message (or at least give me an audio input I could send some beeps over), and then hang up. Given all the medical emergency autodialer products out there, someone must have a bunch of this logic prepackaged in some form or other, but I'm not finding it.
I'm willing to dedicate a BS/2 to this function, so a chip that needs to be told what number to dial, what noises to play, and when to hang up, is fine. I know it's possible to build this out of discrete components, but I'd like something I can absolutely rely on... thanks for any help.

This one can be activated by a dry contact, ie. relay
Control that relay from your javelin and you can send
emergency calls based on data in your javelin.
