Whit made some beautiful custom long sleeve T-shirts for Red Sea Robotics, which was probably a fair amount of work and up-front money. If you want a zero-effort one-off item with your logo or photograph, look no further than your local Walmart photo center. I had a coffee mug done which turned out awesome. They do shirts, too. Just upload your image to their site and click order. Only about $11-20 based on options. I got the "red photo mug" and the image wraps around the whole mug. Delivered for free to my local store in just a couple days.
Another place to have stuff printed with your picture/logo, is Zazzle.com. They'll print on just about anything, and everything I've gotten from them has looked great.
