The old style mains transformer/rectifier type are unregulated and the voltage rating is nominal under load. Unloaded they can read much higher. In other words, this is normal.

    Junk it!

It's not worth the bother to clean up the output when good ones cost so little.

...or cost nothing. In my town, we have an annual Electronics Recycle day (and there are several in surrounding areas at many different times). Most collect some small fee for the big stuff, and let you drop off little stuff for free to keep them out of the landfills. Most of them allow electronics buzzards like myself to scavenge. On average, I pick up 50 to 75 new wall-warts a year, in every voltage from 3 to 48, and more and more, a lot of them are switchers with good voltages & regulation right out of the gate.

If you're really lucky, you can find a few things with steppers, servos and solenoids in them!

I haven't had to buy a wall wart for well over a decade now, and I use a lot.

Should be 9v and it is putting out 13v? FREE ENERGY! Sell it for millions.
