xbee S1 "Failed to enter command mode"

XBee S1 will not respond when seated on the XBee USB adaptor with X-CTU. So far, been able to use another "control" module to test that the USB connection, software, and "Base" profile work fine. In addition, LEDs are connected to Pin 13, Pin 14, Pin 16.

Here are my results:

(2) "Test" XBee: (ATSL40616B04) X-CTU app:
(2.a) API - Not Enabled
RESULTS: Failed to enter command mode, Check connections and try again., Read parameters..Failed.
Pin13: Off
Pin14: On
Pin16: Off (but appears faintly ON)
(2.b) API - Enabled
RESULTS: Failed to enter command mode, Check connections and try again., Read parameters..Failed.

Is this the approach to determine if the module has a sleep mode setting, or burned-out?
