How to "program" Parallax's universal remote?

I have the 020-00001 Universal Remote and while Parallax
was kind enough to post a code sheet, I don't see the instructions on how to correctly enter the codes.
(and of course I misplaced mine. )

Anyone remember how to program this remote?
