surface mount manufacturer?

Can someone suggest a surface mount manufacturer for 1) prototype boards 2) production boards under 1k quantity?

There are many options for pcb board manufacturers but I'm having a difficult time finding one for surface mount. I prototype with through hole but the boards are much nicer if they were surface mount and my hands are not steady enough to hand solder these tiny things nor do I have the microscope, etc.

I assume I just supply the schematics and bill of materials and they build it or is there some sort of GERBER type file for the surface mount that I need to supply too?

Are these production runs relatively expensive processes or are they pretty automated much like CNC machines are?

What country are you in? I've dealt with several in the UK.

The PCB software I use can generate coordinates for a pick and place machine.

i may be a little confused. Are you looking for assembly of your products? If that is the case, usually you provide a bill of materials, materials, and boards to the assembly house. I have purchased parallax's pick and place machine and am going to be offering this service for US customers.
