Here's something that's interesting:
There's a guy named Chuck Moore (the creator of Forth by the way) who designs chips using his custom CAD software written in assembler/forth. He does it at the lowest level: by drawing out every single transistor, layer by layer. Everything done on his tiny custom OS on a really old PC. He claims most tools are too complex and abstract; this gives him a lot of control and simplicity.
Now that's friggin hardcore!

I've read the Propeller was designed in a similar manner.
